PERCEPÇÕES DE USUÁRIAS DO AMBULATÓRIO GINECOLÓGICO SOBRE O ATENDIMENTO EM UM HOSPITAL UNIVERSITÁRIO

Abstract

The medical degree takes place through theoretical and practical training, in which university hospitals are the main scenario of practices. In consultations involving issues of patient intimacy, embarrassment is a possibility. This may compromise the quality of care and/or medical training. The aim of this study was to investigate the psychoaffective aspects of gynecological consultation, based on the perception of patients, in order to find elements that can favor the adequacy of medical education to the humanization of care. This is an observational, cross-sectional, qualitative study, carried out in the gynecology outpatient clinic of a university hospital. Outpatient users who were in the waiting room for gynecological care were included, configuring themselves as a sample for accessibility. Data collection took place between the months of february and august 2021, through semi-structured interviews with previously prepared guiding questions. A total of 50 interviews were conducted, whose discourses were later transcribed and analyzed by the IRaMuTeQ software. From the perspective of the patients, it was found that the moment of the gynecological consultation involves two main spheres of interest: the medical-gynaecological assistance given to them and the teaching and learning aimed at students. Within this division, two perspectives emerge: the one that starts from the personal demand and the one that considers the interest of the other (the student). In the discourses on medical-gynecological assistance, the speeches focus on themselves, on the embarrassment experienced during the assistance. In the discourses on the teaching and learning of the students, the speeches mostly focus on the recognition of the importance of clinical practice in medical training. In this way, there is an intersection between "the care provided to me" and the "other’s teaching and learning process". The embarrassment of the patients seems to be overcome in favor of a greater goal: to contribute to the training of future doctors. It was concluded that, despite the embarrassment experienced in gynecological consultation in an academic environment, the users' understanding of their contribution to medical education promotes a collaborative positioning, revealing itself as a facilitator of care in this scenario. It was also revealed an attenuation of the embarrassment proportional to the experience of women in this type of care.
