IDENTIFICAÇÃO DE PREDITORES ELETROCARDIOGRÁFICOS DA INCIDÊNCIA DE FIBRILAÇÃO ATRIAL PÓS CIRURGIA CARDÍACA

Abstract

Atrial fibrillation is an important comorbidity in the postoperative period of cardiovascular surgeries. The electrocardiogram is a very useful tool in identifying such a complication, being an accessible and inexpensive test. The present study aims to identify predictors in the preoperative electrocardiogram of the incidence of atrial fibrillation in the postoperative period of cardiac surgeries, with a view to better clinical management of the patient. Methodology The present study is observational, analytical and longitudinal. Patients over 18 years of age who underwent cardiac surgery at Hospital Dom Rodrigo, in João Pessoa, were included. Patients were analyzed according to their social data, and their electrocardiograms were analyzed according to the chosen variables. Data were analyzed using the statistical software JASP version 0.16.3 (Apple Silicon). Result A total of 105 patients were included, of which 65 were male and 50 were female. Regarding the electrocardiographic variables, a statistically significant difference was found regarding the amplitude of the p wave, regarding the presence of ventricular extrasystoles and regarding the presence of ventricular overload. Discussion The impact on morbidity and mortality of atrial fibrillation after cardiovascular surgery is significant, with an increase in mortality and thromboembolic events, and prevention tools and protocols are of paramount importance. The main electrocardiographic variable predictive of this complication described in the literature is the duration of the p wave, corroborating the findings of the present study. The identification of these predictors has enabled the emergence of prophylaxis protocols using amiodarone and magnesium supplementation preoperatively.
